I hereby authorize Fix My Playstation (FMP) to replace my iPod Video or
Nano LCD. I will not hold FMP responsible for any items lost or stolen during
shipping. I understand that in order to open and examine the unit, FMP will
break the Warranty Seal thus voiding any remaining manufacturer warranty.
I understand that if the iPod is not economically feasible to be repaired
I will receive a refund minus the $10 return shipping fee, and minus a $25
checkout fee if the Warranty Seal is broken prior to arrival at FMP. I understand
that FMP provides a 90 day warranty on labor and replaced parts on repaired
iPods. No coverage if customer breaks LCD again. However, discounted pricing
offered to FMP customers for 2nd LCD replacement. I understand while FMP
attempts to make same-day repairs, some repairs require extensive labor
requiring additional time or parts that may not be immediately available,
and I will not hold FMP responsible for reasonable extensions of repair
time estimates either expressed or implied. |
