I hereby authorize Fix My Playstation (FMP) to repair my PS2. I will not hold FMP responsible for any items lost or stolen during shipping. I understand that in order to open and examine the unit, FMP will break the Sony Warranty Seal thus voiding any remaining manufacturer warranty. I understand that if the PS2 is not economically feasible to be repaired I will receive a refund minus $20 for exam and return shipping. I understand that FMP provides a 90 day warranty on labor and replaced parts on repaired PS2s, and that my PS2 will be sealed with a tamper-proof warranty seal. I accept full responsibility for any and all damage done to my PS2 if it is opened following repair. I understand while FMP attempts to make same-day repairs, some repairs require extensive labor requiring additional time or parts that may not be immediately available, and I will not hold FMP responsible for reasonable extensions of repair time estimates either expressed or implied. |
